.Samantha's Closet 1st Annual "Lace Wig Expo" Giveaway will be awarding a
free lace wigs plus an adhesive starter kit to an extraordinary woman who is
experiencing hair loss due to Cancer, Alopecia, Trichotillomania or any other
condition that causes hair loss.

If you know anyone experiencing hair loss due to the above conditions,

win a free full lace wig courtesy of Samantha's Closet.
Before entering the contest, please scroll down and make sure you read the
terms and conditions of this campaign. One entry per person.

Trichotillomania Groups and Treatment Centers - if you would like to receive
Free Entry Forms to display in your office and share with your patients, please

Who Can Enter
1. To be eligible to participate in this campaign giveaway, entrants must be U.S.
residents who are suffering from hair loss due to Cancer, Alopecia,
Trichotillomania or other conditions that cause hair loss. The minimum age
requirement for this contest is 18. IMPORTANT: Entries from women not
suffering from these conditions will not be accepted for this particular
campaign.
2. Employees of Samantha's Closet [and any other company sponsoring the
contest], their affiliated companies, advertising and promotion agencies, and
the immediate family members of each are ineligible. The term "immediate
families" includes spouses, grandparents, parents, siblings, children and
grandchildren.
Requirements of Winners
By entering this drawing the winners agree to the following terms:
1. All winners must agree to take Before and After Photos along with a
Testimonial.
2. All winners must agree to submit the Before and After Photos along with a
Testimonial to Samantha;s Closet so that we can publish them on the campaign
winners page of our website at www.samanthascloset.us. These photos must
be of good quality and must be a clear and close up head shot.
3. All winners agree to submit a Before Photo of themselves prior to receiving
their full lace wig within 10 business days of receiving their winner notification.
This can be a photo of your natural self or one of yourself featuring the wig or
hair style that you presently wear.
5. Once the Before Photo is received, your free full lace wig and kit will ship out
to you within 1-3 business days.
4. All winners then agree to submit their After Photo within 10 business days of
receiving the wig. These photos must be of good quality and must be a clear
and close up head shot.
5. Both photos will be featured on our website under the campaign winners
page.
6. The Before Photos will be placed on the website immediately upon receipt.
7. The After Photos and Testimonial will be placed on the website within 1-2
business days of receipt.
8. The winners will send their Before and After Photos along with a Testimonial
to fabweavesntheville@samanthascloset.us
8. Winners are not required to come into the office to pick up their full lace
wig, it can be shipped to you via US Mail with signature required.
How to Enter
1. Complete the entry form online
2. No purchase is necessary to enter.
3. One entry per person.
(a) Multiple entries will be discarded.
(b) Multiple e-mail addresses from the same IP is considered SPAMMING, and
will not be acceptable.
(c) All entries will have their IP addresses monitored and recorded. Participants
not adhering to these rules will be permanently deleted from the drawing.
4. To enter, fill out the online contest entry form. Make sure you fill out the form
with a valid e-mail address and telephone number. This is the information that
we will use to notify the WINNERS. If the information is incorrect, another
randomly selected WINNER will be chosen. So, make sure to properly fill out the
entry form.
5. The contest starts on October 7, 2009 and all entries must be received by 5
p.m. ET on November 3, 2009. Samantha's Closet is not responsible for lost,
late, or misdirected mail errors. The Winner will be selected by random
drawing from among valid entries and announced on November 7, 2009. The
WINNERS will be notified by the e-mail and telephone number that they provide
on the entry form.
The Prize
1. One winners will be selected. The winners will receive: A Lace Wig, An Outfit
2. Winner will be notified by e-mail and telephone. A return response via e-mail
and/or telephone from the winner will be required in order to ship the prize.
3. Failure to respond to our notification within 10 business days will constitute
forfeiture of the prize. If forfeited, the prize will remain the property of
Samantha's Closet and another winner will be drawn.
4. There will be no substitutions or cash alternatives for prizes.
5. The winners will receive their free full lace wig and adhesive starter kit via
UPS with Signature required - we will not ship to P.O. Boxes.
6. Only one prize will be awarded per entrant.
7. Samantha;s Closet is not responsible for any liabilities arising directly or
indirectly from the award or use of the prize.
8. In claiming your prize, you acknowledge that Samantha's Closet will have the
right to publicize your name, and the fact that you are the winner for
promotional purposes without financial remuneration.
Odds of Winning/Miscellaneous
1. Odds of winning depend on the number of [correct] entries received.
2. This contest is subject to all applicable laws and regulations and is void
where prohibited. Samantha's Closet [and any other sponsors]
[accepts/accept] no responsibility or liability in connection with any injuries,
losses or damages of any kind caused by or resulting from the acceptance,
possession or use of any prize awarded hereunder.
3. By registering in our contest, you agree to receive promotional e-mail offers
